Advanced Care Planning - Free information session

As dementia progresses, people may no longer be able to make decisions on their own regarding the healthcare they would like to receive in the future. It might be hard to think about, but by planning ahead and communicating decisions, you can ensure your wishes are known. It can also be daunting if you know someone who is now unable to make these decisions and you may have to make choices on their behalf as the enduring guardian.

The goal of the workshop is to talk more at length around the following topics:

  • what you want loved ones to do if you could no longer makes those decisions yourself
  • difference between an Advance Care Directive and Advance Care Plan
  • what is capacity and what does this mean for future planning
  • how dementia advances
  • how to build the advance care plan

This session is run by two Clinical Nurse Consultants from the charity Dementia Australia who are part of the Nightingale Program; an initiative focusing on Dementia specific palliative support in the community.

There will be a presentation and an opportunity to ask questions afterwards.
